In order to provide our patients with the best care possible, please provide all of the following information at your scheduled appointment. If any of these items are not supplied at the time of your visit, you will be rescheduled.

• Completed patient information packet
• Photo ID
• Insurance card
• Referral (if needed)
• Minors (under the age of 18) must have a parent present at their visit, or a letter stating it is okay to be seen without parent, with parents signature and photo ID.
